JOB DESCRIPTION

Outreach Worker

Lifeworks Staffordshire requires an experienced outreach worker to work with the Project Lead for the lottery funded project There4U. Having knowledge and/or experience of  the family of conditions the charity works with as identified in the DANDA model  updated by Lifeworks Staffordshire 2024. With the main focus on supporting clients with an average or above IQ with Autism, ADHD and secondary mental ill health, on a one -to -one basis in the local community of North Staffordshire.

Work activities include:

• To work with the Project Lead to discuss clients and carers requirements.
• To develop a positive professional working relationships with community based clients, carers and professionals.
• To assist clients to achieve their goals through identifying and addressing their areas of need
• To Signpost carers for support both within the charity and external to other organisations.
• Keep up to date with government legislation and charity policies, areas of development for conditions (as identified in the DANDA model 2024) 
• Oversee the gathering of information for evidence/outcomes by completing the charity’s database and undertaking any other administrative duties commensurate with this post.

Terms and Conditions

Will be DBS checked

Hours of Work 20 Hours covering Monday to Friday with occasional weekend working when required

Holidays; statutory plus bank holidays (pro rata)

Salary £ 15,000 per annum 

Contract fixed term for 3 years

Must have a driving licence and access to a car and have knowledge of the North Staffordshire conurbation   . 

Previous Experience 

Working mainly with Asperger's Syndrome, high functioning Autism,ASD, ADHD and Dyspraxia. Secondary mental ill health 

Skills: Critical thinker, calm, effective communicator, good written skills, computer literate.
